Effective Date: September 1, 2025

At Angelica Janik Wellness Services, your privacy and confidentiality are taken seriously. This Privacy Policy outlines how your personal information is collected, used, stored, and protected in compliance with applicable privacy laws, including Alberta’s Personal Information Protection Act (PIPA). This policy applies to all in-person and Telehealth services provided through this practice.

By engaging with these services, you consent to the practices described in this policy. If you have any questions or concerns, you're encouraged to reach out.

1. Collection of Personal Information

Personal information is collected to provide effective and ethical psychological services. This may include, but is not limited to:

  • Personal Identifiers: Name, address, phone number, email, and date of birth.

  • Health Information: Relevant medical history, mental health background, treatment goals, and session notes.

  • Telehealth Information: Date/time of virtual sessions, video/audio data, and correspondence related to online care.

  • Payment Information: Insurance details, billing address, and payment method (where applicable).

Only the information necessary for assessment, treatment, and communication will be collected—either verbally or in writing.

As a contractor working out of various Calgary clinics, all personal and health information will be stored securely on the electronic platform used by the clinic location you choose to attend. Upon your first session, all required intake paperwork will be completed and stored directly through that clinic's system. This ensures your information remains centralized, protected, and in line with the clinic’s data policies.

2. Use of Personal Information

Your information may be used for the following purposes:

  • Psychological Services: To assess, treat, and support your mental health goals.

  • Administrative Functions: Appointment booking, billing, file management, and follow-ups.

  • Legal Obligations: Compliance with mandatory reporting laws and regulatory requirements.

  • Telehealth Delivery: Providing virtual care using secure and encrypted platforms.

  • Website Optimization: Improving website performance and user experience.

Your information will not be used for any other purposes without your explicit consent, unless required by law.

3. Consent and Informed Consent

By sharing your personal and health information, you consent to its collection and use as described in this policy. Prior to beginning therapy, assessments, or telehealth services, informed consent will be obtained—either in writing or verbally. This process includes a clear explanation of the nature of services, potential risks and benefits, limits of confidentiality, and your rights. Consent for telehealth services will specifically be obtained before any virtual sessions begin. You may withdraw your consent at any time with reasonable notice, subject to legal or contractual obligations.

4. Protection of Personal Information

Reasonable safeguards are in place to ensure the security and confidentiality of your information:

  • Paper files are securely stored in locked spaces.

  • Electronic records are encrypted and stored using secure systems.

  • Telehealth is delivered through privacy-compliant platforms.

  • Access to your information is limited to those directly involved in your care.

While all efforts are made to maintain security, no system is completely immune to risk. Please speak with your clinician if you prefer alternative arrangements.

5. Email Communication

Please be aware that email communication is not fully secure. While reasonable precautions are taken, clients are encouraged to avoid sending sensitive personal or clinical information via email. For matters requiring confidentiality, more secure forms of communication can be arranged.

6. Accessing and Correcting Your Information

You have the right to request access to your personal and health information, which will be provided within 30 days of your request unless disclosure could cause significant harm to yourself or others, as outlined in the College of Alberta Psychologists (CAP) guidelines. If you find that any information is inaccurate or incomplete, you may request corrections. To make access or correction requests, please contact Angelica Janik directly.

7. Disclosure of Personal Information

Your personal information will not be disclosed without your written consent, except in the following cases:

  • With Other Healthcare Providers: When coordination of care is needed and you provide consent.

  • Legal Requirements: In cases such as suspected abuse, imminent harm, or legal subpoenas.

  • Health Insurance Purposes: For claims or billing purposes, with your prior authorization.

  • Clinical Supervision: As a registered provisional psychologist, clinical supervision is a regulatory requirement. Information relevant to your case may be discussed with a registered supervisor to ensure ethical and effective care. Identifying information is minimized wherever possible.

8. Breach Notification

If a data breach occurs affecting your personal information, I will promptly notify you and comply with Alberta’s privacy laws and directives from the Information and Privacy Commissioner.

9. Retention of Personal Information

Records are retained for a minimum of 10 years following the last date of contact or, in the case of minors, 10 years past the age of majority, in accordance with professional and legal standards. All records are stored securely, and when no longer needed, are destroyed in a manner that protects your privacy.

10. Telehealth Services

Telehealth is provided through secure, encrypted platforms to protect your privacy. However, some inherent risks—such as connectivity issues, unauthorized access, or technological failures—may still exist.

Depending on the clinic location you choose, virtual sessions and electronic record keeping will be managed through that clinic’s secure platform. Each clinic is responsible for maintaining the confidentiality and security of your data within their system.

By choosing to participate in virtual therapy, you acknowledge and accept the associated risks. If you have any concerns about online sessions or data security, please don’t hesitate to reach out to me directly for more information or to discuss alternative options.
